It is all about elastic, the pouch would have to be essentially self supporting like a cock sock, the fabric connecting to the waist at the side and the rear is mostly for show.

I personally don't like that suit but I do have the one sided thong. First, you have to be shaved, The suit needs to get a grip on your package, second, it stays on very well with no inner loops or elastic. It isn't a suit that you are going to dive in a pool or wear in the surf. However for calm waters it is a fun suit. It seems to defy logic and gravity. It is designed in such a way that it must go over the left hip. I found out that if you turn it wrong side out you can wear it over the right hip so you you can get an all over tan and keep the jewels protected from a burn. Its great for tans and to catch the eye of others --I love mine.
You will find that Viseau also makes that one sided thong (theirs with a buckle). If you don't trust the staying power of the suit check out Body Aware, they have the suit with a thin string to secure it. Either way, not for the faint of heart or those that fear a "wardrobe malfunction"

Where does it come from? Strange pic - the guy in the black speedo is in focus but he is surveying some fuzzy area ahead in white.
